ABSTRACT

Febrile seizure(FS) is generally defined as seizures occurring in children typically 6 months to 5 years of age in association with a fever greater than 38℃, who do not have evidence of an intracranial cause, another definable cause of seizure, or a history of an a febrile seizure. At present, it is considered that FS is caused by multiple factors, mainly duing to the developing central nervous system is vulnerable to fever and potential risk factors (including environmental and genetic factors). According to the type of seizures, it can be divided into simple FS and complex FS, distinguishing the type will be helpful to formulate clinical intervention strategies.Benzodiazepines can quickly stop convulsion, and it is mostly used for patients with a duration of more than 5 minutes. At the same time, intermittent diazepam treatment can effectively reduce the risk of recurrence of FS and relieve parents′ anxiety and fear. This review briefly summarized the risk factors, clinical evaluation and treatment of FS in order to improve understanding.

ABSTRACT

Objective: To study the apoptosis mechanism of microwave ablation (MA) in tongue cancer Cal-27 cell transplanted tumor in nude mice. Methods: 20 nude mice with Cal-27 cell subcutaneously transplanted tumor were randomly divided into blank control group, 50 ℃ 1 min MA group, 50 ℃ 2 min MA group and 55 ℃ 2 min MA group (n = 5) respecitvely, and were treated with MA with indiated perameters respectively. The apoptosis of tumor cells was observed by HE staining. The expression of Caspase-3 and PCNA protein was detected by immunohistochemical SP method. Results: HE staining showed that microwave ablation induced apoptosis of the tumor cells. Immunohistochemical results showed that the expression of Caspase-3 protein was significantly up-regulated in the microwave ablation groups compared with the control group (P < 0. 05), and the expression of PCNA protein was significantly decreased (P < 0. 05) . In microwave ablation 55 ℃ 2 min group, the expression of Caspase-3 protein was increased more (P <0. 01), and the expression of PCNA protein was decreased more (P < 0. 01) . Conclusion: Microwave ablation may inhibite the proliferation of tongue cancer cells by inducing apoptosis of the cells. The mechanism may be related to the up-regulation of Caspase-3 signaling pathway and down-regulation of PCNA signaling pathway.

ABSTRACT

OBJECTIVETo compare the outcomes of hand-assisted laparoscopic liver surgery (HALS) and pure laparoscopic liver surgery (PLS).

METHODSThe clinical data were analyzed for 64 patients undergoing major hepatectomy with HALH (23 cases) and PLS (41 cases) between January, 2010 and December, 2012.

RESULTSThe general data of the two groups were comparable. Compared with PLS, HALS was associated with a significantly shorter operative time (240 vs 191 min), less intraoperative blood loss (430 vs 220 ml, P<0.05), and a lower cost (P<0.05). There was no significant difference between the two groups in postoperative hospital stay, complication rates or recurrence rate of hepatocellular carcinoma.

CONCLUSIONHALS is safe for major liver resection with such advantages over PLS as causing less trauma and a lower cost.

ABSTRACT

Objective Investigations regarding the status of pediatric emergency and relevant facilities distribution in our province represent a crucial prerequisite for further setting up emergency network and improving our first aid capabilities. Methods We had conducted relevant surveys on the medical institutions within our province, including lever 2 and lever 3 hospitals as well as maternal and child care service centers. Results The feedback from 151 institutions indicated that they had 4485 pediatric beds and treated 5 308 severe patients in 2010.Among all,38 hospitals had ventilators for children and 4 hospitals could perform pediatric blood purification treatment. However,no one had mastered the children ECMO technique. Conclusion There is an inadequacy of some emergency equipments and the development in emergency and first aid is uneven. There is a strong request to launch and develop the blood purification and bronchfiberscope techniques,in combination with improvements in the first aid capabilities and emergency network.

ABSTRACT

@#Objective To determine the effect of transcranial Doppler ultrasound (TCD) combined with focal mild hypothermia on cerebral infarction treated with thrombolytic unokinase (UK).Methods32 rabbits were randomly divided into the TCD group, UK group, TCD+UK group and hypothermia group with 8 animals in each group. Mean velocities of the middle cerebral artery were recorded after unokinase infusion to estimate whether blood vessels reopened. Meantime, brain temperature of lesion site was tested to find out whether declined to hypothermia level.ResultsThe percentage of recanalization was 87.5% in the hypothermia group, 75% in the TCD+UK group, significantly higher than that in the UK group (37.5%)(P<0.05). After 30 minutes of the rabbit refrigerated at 2 ℃, the putamen temperature declined to 32 ℃~34 ℃.ConclusionApply the regional refrigerstor apparatus to decline the brain temperature to mild hypothermia level during therapeutic time window. TCD combined with focal mild hypothermia can improve the thrombolytic effect of unokinase.

ABSTRACT

BACKGROUND: Although angiography is the gold standard for the diagnosis of cerebrovascular disease, it cannot be used widely because of its traumatotaxis. Transcranial Doppler ultrasound (TCD) and MRA are both non-invasive examinations, which can be used to evaluate intracranial large artery vascular trunks and hemodynamics.OBJECTIVE: To investigate the reliability of diagnosis with TCD on hemodynamic changes of ischemic cerebrovascular disease (ICVD) and the relativity between TCD and MRA.DESIGN: Case analysis based on patients.SETTING: Neurological Department of the First Affiliated Clinical Hos-pital, Harbin Medical University.PARTICIPANTS: From April 2001 to February 2002,totally 45 patients with stroke, selected from outpatients and inpatients of Neurological Department of First Affiliated Clinical Hospital of Harbin Medical University, examined by CT, to be told the fact and accepted, were taken part in the study.METHODS: Patients were examined with TC-2021 TCD instrument produced by German EME Corporation and VISART1.5T superconduct MR instrument produced by Toshiba. Totally 45 patients had TCD and MRI examinations, and the alternation between two examinations was 1 to 15 days. The hemodynamics and morphological changes were observed including middle cerebral artery (MCA), end of internal carotid artery (ICA),anterior cerebral artery (ACA), posterior cerebral artery (PCA), basal artery (BA), and vertebral artery (VA).large artery vascular trunks and hemodynamic results with TCD and MRA;ty, false positive and false negative with TCD with those of MRA.TCD and MRA: Theory data was Tmin=9.91; χ2=107.92, P < 0.005, it was considered that the results with the rate of TCD were related with that of MRA. The detection rate of TCD was 19.06%, and that of MRA was 15.25%. There were significant differences between the detection rate of standard, the sensibility of diagnosing abnormal vessels by TCD was 78.10%, specificity 94.22%, false positive 7.46%, false negative 23.31%,and the coincidence rate 89.36%.CONCLUSION: TCD can assess exactly the functional status of the vessels. It can also reflect early and sensitively the change of hemodynamics of cerebral blood flow. While MRA can display the change of vascular appearance directly so that the combination of TCD and MRA will be improve sensibility and specificity of diagnosis.
